Looks like a variation of the anal pear, the infamous tool of the Spanish Inquisition! It was also called the “pear of anguish”. In its traditional form, it was a simple screw device with three curved iron petals. Screwed closed, it was roughly the shape of a butt plug, or, as the name implies, a pear. Closed, it could be inserted in the anus, the vagina, or the mouth. And then, of course, the inquisitor would start turning the screw, causing the petals to be forced apart. Let’s all shudder together at the thought! What horror and brutality is enforced in the name of religion!
To be fair, it was also used in the Middle Ages to torture women who suffered a miscarriage, were liars, blasphemers and homosexuals.
